*Some insurance companies may have a behavioral health “carve-out”. In those cases, your behavioral health benefits are managed by a separate company with its own network of providers. Dr. Nardine may or may not be a provider in the carve-out network. if you are unsure whether or not your insurance has such a carve-out, please contact our insurance company your employer.

**Contact your insurance company to determine if your plan requires prior authorization.  If this is needed, call your primary care provider before your first appointment.  If an authorization is required by your insurance, and it is not obtained, you will be responsible for payment.
